1. Services Provided

OnDemand Mobile Lab offers mobile phlebotomy and laboratory collection services, including specialty services such as therapeutic phlebotomy, IV therapy support, diagnostic specimen collection, and physician-ordered procedures. Services are only provided with appropriate physician orders where required by law.

2. Eligibility

You must be at least 18 years old to schedule or consent to services. For minors, a parent or legal guardian must authorize and be present during service delivery.

3. Medical Disclaimer

• OnDemand Mobile Lab does not provide medical diagnosis or treatment.
• Our services are limited to specimen collection and laboratory support as      prescribed by your licensed healthcare provider.
• Always consult with your physician regarding diagnosis, results, or treatment decisions.

4. Scheduling & Cancellations

• Appointments may be scheduled via phone, text, or our website.
• Cancellations must be made at least 24 hours in advance. Missed appointments or late cancellations may result in a service fee.
• We reserve the right to reschedule appointments due to staff availability,      safety, or unforeseen events.

5. Fees & Payment

• Service fees are due at the time of service unless otherwise arranged.
• Some services may not be covered by insurance. Patients are responsible for confirming coverage with their provider.
• Additional fees may apply for urgent requests, after-hours services, or travel outside designated areas.

6. Patient Responsibilities

By using our services, you agree to:

• Provide accurate personal, medical, and insurance information.
• Follow your physician’s orders regarding lab tests or procedures.
• Treat our staff respectfully and ensure a safe environment for service delivery.

7. Privacy & Data Protection

8. Limitation of Liability

OnDemand Mobile Lab and its staff shall not be liable for:

• Delays in test results caused by third-party laboratories.
• Medical outcomes or treatment decisions based on test results.
• Any indirect, incidental, or consequential damages related to our services.

• The website content is for informational purposes only.
• Unauthorized use, scraping, or reproduction of website content is prohibited.
• We may update or remove content without prior notice.

10. Governing Law

These Terms are governed by the laws of the State of Texas. Any disputes shall be resolved in the courts located in Denton County, Texas.
